Have Medicaid, Lives in Indiana, Need help with day-to-day activities, Able to direct your own services (if not, have an individual representative to appoint), 60 and over and qualifies for the Pathways MLTSS Waiver, 59 and under and qualifies for the Health & Wellness (formerly Aged & Disabled) Waiver
How it works?
Who can be a caregiver? Someone who is at least 18 years of age, Someone who is eligible for work in the United States, Someone who is not excluded from the Medicaid program The Caregiver may be a non-family member or a family member who lives with the Participant in the private home of the Participant or the Caregiver.
